My Lupus Diary 會員計劃 Membership Programme 

MLD會員計劃將會不定期推出不同的活動，分享專業資訊，提供不同的優惠供會員享用。 紅斑狼瘡患者、照顧者以及關注紅斑狼瘡症的大眾均可參加。 所有收入扣除營運費用後，將會全數撥入MLD的營運資金。

 

The MLD Membership Program organizes different activities to share professional advice and provides various benefits for members. Lupus patients, caregivers, and the general public who concern about Lupus can participate in the program. All the revenues raised will be donated to MLD's operational fund after deduction of costs.

  • Type 1 第一類

  • - SLE Patients 紅斑狼瘡症患者

  • - Patients with other Autoimmune diseases 其他免疫系統疾病患者
     

  • Type 2 第二類 

  • - Relatives / Friends of Patients 患者親友
     

  • Type 3 第三類 

  • - People who concern about and interested about SLE 關注紅斑狼瘡症的大眾

All memberships are valid from April 1 to March 31 of the subsequent year.

會費有效期為每年之四月一日至次年之三月三十一日止。

Arrangement options for welcome gifts delivery: 會員入會禮物交付，可選以下安排：

1). SF delivery borne by member. 安排順豐到付，運費由會員承擔。

2). Self pick up during MLD event. 於MLD活動自取。

3). Self pick up at MLD office (Please contact us for an appointment) MLD辦公室自取（請聯繫我們以便預約）

個人會籍

The membership is valid from 1st April of each year to 31st March of the following year. 
會籍有效期為每年之四月一日至次年之三月三十一日止。

未滿十八歲的申請人

  • If the applicant is under 18 years of age, the applicant must obtain consent from his/her parents or guardian, nominate such an individual as his/her emergency contact person and provide his/her contact numbers.  未滿十八歲的申請人，請先徵得合法監護人的同意及指定其為你的緊急聯絡人，並提供其聯絡電話。

退會申請

更改個人資料

  • If you need to change your personal information (mailing address, phone number, fax, etc.), please do so as soon as possible by emailing mylupusdiaryhk@gmail.com in writing, to ensure that members can receive the latest information from the association.

  • 如需更改會員個人資料（通訊地址、電話、傳真等），必須盡快以書面電郵至mylupusdiaryhk@gmail.com ，確保會員能收到本會之最新資訊。
